How to Obtain Your Eaton County Vital Record
BIRTH RECORDS are recorded in the county where the birth took place and are PRIVATE RECORDS.
To obtain your record, please provide-
Full name at birth
Date of birth
Parents names, including maiden name of mother
Photo copy of your driver’s license or state I.D.
Signature
Note: Records will be mailed to the address shown on your pictured I.D., so if you have a change of address on the back, make a copy of it. Otherwise, for a second piece of evidence of your current address you may provide a utility bill or medical record.
DEATH RECORDS are recorded in the county where the death took place. These records are public records.
To obtain one please provide-
Name of deceased
Approximate date of death
MARRIAGE RECORDS are recorded in the county of residence at the time of your application. These are public records.
To obtain this records, please provide -
Name of each applicant at the time of application
Date or year of marriage
FEES - $12.00 For 1st certified copy, $5.00 for each additional copy of the same record. Payments in the form of a check or money order can be made out to the EATON COUNTY CLERK. You should receive your request within a week.
