Passport Information

 

   Applications are taken 8:00am to 4:30p only.

We do not take credit cards 

 

PASSPORT FEES INCREASE TUESDAY, JULY 13,2010

 

 

*Applicants age 16 and over must pay $100.00 for their passport.  Passports are for travel anywhere in the world.  This fee includes a $75.00 check or money order to the U.S Department of State and $25.00 check, money order, or cash to Eaton County Clerk. WE ARE NOT A FEDERAL AGENCY SO, SEPARATE CHECKS ARE REQUIRED.  The passport is valid for 10 years.

*Passcards are also available for travel by land to Canada, Mexico and by Sea.  Applicants 16 years of age or older must pay $45.00 for their passcard.  This fee included a $20.00 check or money order to the U.S. Department of State and a $25.00 fee to the Eaton County Clerk.  This passcard is valid for 10 years.                                                                                        

*Applicants age 15 and under must pay $85.00 for their passport. This includes a $60.00 check or money order to the U.S. Department of State and $25.00 check, money order, or cash to Eaton County Clerk.  SEPARATE CHECKS ARE REQUIRED.  The passport is valid for 5 years.

*Passcards for ages 15 and under must pay $35.00.  This fee includes $10.00 by check or money order to the U.S. Department of State and a $25.00 check or money order to the Eaton County Clerk.  The passcard is valid for 5 years.

 

Please bring the following items when applying for a Passport.

 

Application, birth certificate with a raised seal and valid, up to date identification.

*THE BIRTH CERTIFICATE WILL BE RETURNED AFTER YOU RECEIVE YOUR PASSPORT.

 COMPLETED APPLICATION (DSP-11)
*You can obtain the Form DSP-11 from your passport acceptance facility in Eaton County or from the Internet
   *Type or print in black or blue ink only.
   *DO NOT sign application until given oath by clerk.

 EVIDENCE OF CITIZENSHIP (one or more of the following may be required)
*Certified copy of birth record from the State Vitals Office or from the County your were born.  This will prove your U.S. Citizenship (Additional documentation may be necessary for persons that have been adopted or have had a legal name change or have a delayed birth record.
   *Previously issued US Passport
   *Original Naturalization Certificate

 IDENTIFICATION

*Persons ages 16 and older can apply without a parent but must present a valid Drivers License or State issued Identification Card at time of application and use a parent as an emergency contact.  *Persons ages 15 and under must be accompanied by two parents listed on child’s birth certificate. Parent must present a valid Drivers License or State issued Identification Card at time of application.

 TWO DUPLICATE PASSPORT PHOTOGRAPHS

We take passport pictures here at our office for $10.00.  *2 x 2 inch Photographs must meet Passport Services requirements.  *Photos must be taken within 6 months of application.

 

EXPEDITED SERVICE 

IF YOU ARE TRAVELLING WITHIN 3 WEEKS, WE HAVE Expedited service for an additional $60.00.  Approximate processing time is 2-3 weeks.

 Your travel agent or airline can also provide you with information.

  

IF YOU ARE TRAVELLING LESS THAN 2 weeks of application, it is recommended that you go to the nearest passport agency to apply, which is:

The agency is open Monday – Friday from 9:00 a.m. – 4:00 p.m. Central Time and will only offer service by appointment.  Customers may schedule an automated appointment 24 hours a day, seven days a week by calling:  (312-341-6020).  You will need to present proof of immediate travel (airline ticket, confirmed airline-generated itinerary or a letter from an employer for business travel along with completed application, two identical recent photographs (2 x 2 inches), evidence of U.S. citizenship and appropriate fees).


How to Renew a Passport by Mail:

We don’t offer this service but you may renew by mail:  (Form DSP-82) Renewal fee is $75.00.

 Your previous passport must have been issued on or after your 16th birthday.

 Your passport must have been issued within the last 15 years.

 If your name has been changed due to marriage or by court order, you must submit proof of the name change.

 If a passport was issued as a child, that passport cannot be used to renew for an adult passport.
