Doing Business As (DBA) or Assumed Name
- $10 filing fee
- Payments can be made by cash, check, money order or card (additional fees for card apply)
- Certificate of Assumed Name is valid for 5 years.
- Application for a DBA may be made by mail or in person
- Have your signature(s) notarized (sign in front of a notary)
- Notary service is available for no charge at the Eaton County Clerk’s Office, a government-issued identification is required
- For mail-in fill out the form: Certificate of Assumed Name (DBA) or come in person to the clerk's office
- Consent to Service of Process (PDF) - Required for out of State Residents' Filing

- Notary service is available for no charge at the Eaton County Clerk’s Office, a government-issued identification is required
- All partners must sign the form, but the signature of only one partner is required to be notarized.

- You can submit your renewal up to 90 days in advance of the expiration date.
- Owners on the renewal filing must match the last previous registration to be accepted. If the certificate has already expired, for filing purposes, it would be treated as a new registration.
- Have your signature(s) notarized (sign in front of a notary)
- Notary service is available for no charge at the Eaton County Clerk’s Office, a government-issued identification is required
(Our office will send out a renewal notice between 30 and 60 days prior to expiration.)

- Dissolution for DBA, Assumed Name MUST be signed by all owners listed.
- Dissolution for Co-Partnership only ONE of the partners is needed to dissolve.
